Deadlines: This form must be completed by March 23, 2018 for those wishing to begin thesis in May or September 2018.

If you miss this deadline, you may not be assigned a supervisor and committee member of your choice.

The MFA thesis committee consists of two committee members: one supervisor and one additional faculty member. Since supervisors and committee members can be in high demand, it is highly recommended that students indicate more than two choices for committee member. Please note that only tenured or tenure-track faculty as listed below are available as thesis supervisors. In addition to these faculty, lecturers may also serve as committee members. Please read the “Thesis Committee Selection” section of the MFA Handbook for further details.

Please note: your personal information is collected under the authority of section 26(c) of the Freedom of Information and Protection of Privacy Act (FIPPA).
